Heat illness is not a single event but a spectrum, and each stage is more serious than the last.

It starts with heat cramps and heat exhaustion, where your body is straining but still coping.

At the far end sits exertional heat stroke, a medical emergency defined by a core temperature above 104 degrees Fahrenheit combined with confusion or collapse.

What Are the Signs of Heat Exhaustion When Running?

Heat exhaustion is the stage where your body is overwhelmed by heat but your brain is still working normally.

Your core temperature is usually elevated but has not crossed into the dangerous range, and the signs build gradually rather than all at once.

Watch for these symptoms on a hot run:

  • Heavy sweating paired with clammy, pale skin
  • Dizziness or lightheadedness when you slow down or stop
  • Nausea, headache, or a sudden loss of appetite
  • Muscle weakness and legs that feel heavy out of proportion to your effort
  • A racing heart rate that stays high even as you ease off the pace

The moment you notice these signs, stop running and get out of the sun.

Move to shade or air conditioning, loosen or remove extra clothing, and start cooling your skin with cold water or ice.

Sip cool fluids if you can keep them down, and stay put until you feel steady again.

Do not talk yourself into finishing the run, because pushing through heat exhaustion is how runners slide into heat stroke.

How Do You Know If It Is Heat Stroke, Not Heat Exhaustion?

The line between heat exhaustion and heat stroke comes down to two things: how high the core temperature climbs and whether the brain starts to fail.

The skin is often hot and can be either sweaty or dry, so do not wait for dry skin before you act.

What to do if you suspect heat stroke

Call emergency services immediately, then start cooling the runner without waiting for help to arrive.

The fastest method is cold water immersion, which means getting the person into a tub or container of cold, ice-filled water up to the neck.

If immersion is not possible, douse them with cold water and pack ice against the neck, armpits, and groin where large blood vessels run close to the skin.

Why Are Shorter, Faster Races Riskier for Heat Stroke?

Heat stroke shows up more often in fast, short races than in marathons, which runs against intuition.

You would expect the longer time in the heat to be the danger, yet the opposite is true.

The reason is intensity, because a fast 5K or 10K produces far more heat per minute than a marathon run at a steadier effort.

Work from the University of Cape Town showed that runners unconsciously slow down as body heat rises, easing the pace before core temperature reaches dangerous levels.

At marathon pace, heat builds slowly enough that this built-in brake has time to work.

In a short, all-out effort, heat production is so high that a determined runner can override that brake and keep pushing past the point of safety.

The Falmouth Road Race, a seven-mile summer event, is known for producing a cluster of heat stroke cases most years, exactly the hard, hot effort where that brake fails.

The faster and hotter the race, the more important it is to run by feel and back off when your body signals it has had enough.

What Puts a Runner at Higher Risk of Heat Illness?

Heat stroke can strike a fit, healthy runner, but several factors stack the odds against you.

Most of them are within your control, which means you can lower your risk before you ever lace up.

  • Heat and humidity that spike suddenly, before your body has had a chance to adjust
  • Poor heat acclimatization, common on the first hot days of the year or after travel to a warmer climate
  • Dehydration going into the run, which shrinks blood volume and hampers sweating
  • A recent illness or fever, which disrupts the body’s temperature control for days afterward
  • Hard, fast efforts like races and tempo runs, where heat production is highest
  • Stimulants and certain medications that raise body temperature or blunt your sense of strain

Running while sick deserves extra care, because your thermostat is already compromised.

If you are fighting a fever or a stomach bug, skip the hard workout and the hot-weather race entirely.

How Do You Prevent Heat Illness on Hot Runs?

Preventing heat illness comes down to giving your body time to adapt and respecting the conditions on the day.

Start with heat acclimatization, which is the single most protective step you can take.

Research on heat adaptation shows that gradual exposure over ten to fourteen days trains your body to sweat sooner and cool more efficiently.

Ease into the first hot week of the season with short, easy runs rather than jumping straight back into hard training.

On hot days, run by effort instead of pace, and accept that the same effort will feel slower when it is warm.

Shift hard workouts to early morning or evening, when the sun is low and the air is cooler.

  1. I’ve been waiting for an article like this to put my two cents in. Last year I read all about running in the heat, pre-cooling, cooling studies, etc. on your site and one really interested me. Studied subjects run with some sort of cooling gel pads in their hands and there was no noticeable effect on their performance.

    To make it short, I got to running with a frozen half liter bottle of water in my hand and it was a godsend all summer. I definitely
    noticed increased performance even during predawn when I usually run my 9 miles. And it makes one feel so much better during the most obnoxious summer heat.
    Toss a bottle full of water into the freezer twelve hours or so before a run and then enjoy. I sometimes juggle the bottle from hand to hand if it gets too cold or toss it into my running pack. Drink it slowly while it melts and squeeze the bottle to make full contact with remaining ice and prevent annoying rattling. After two thirds of a nine mile run in something like 20 – 23 C heat all I have left is ice water. I’ve already used this method this year and the one time I run in moderate heat without ice I felt much worse. As an added bonus, an ice brick in your hand really wakes you up.
    Hope somebody can use that tip.
